Advanced Health Care Directive Workshop May 25th


Thursday, May 25, 2017, 6 p.m. at the Jackson Memorial Library

A workshop designed to help you make your end-of-life care instructions clear – to family members, your health care team, and emergency medical personnel.

Sarah Dwelley, R.N., of the Pen Bay Palliative Care Program, will discuss end-of-life care and treatment options, and walk us through the Advanced Health Care Directive paperwork which designates a health care proxy and instructs family members and health care providers on the decisions you have made about the kind of care you want – and do not want – in a health crisis.

Ms.  Dwelley will also present information on the palliative and hospice care available in our community to individuals with chronic conditions or illnesses as they approach the end of their lives.

This program is limited to 12 individuals so that we can speak privately with certified health care workers who can assist us in completing the Advanced Care Directive paperwork.   Please contact the library to sign up.
